Waiting, waiting, waiting...

Tomorrow is our appointment with the new RE.  I'm trying not to get excited about it because I just don't want to be disappointed again.

Anyway, I thought I'd write out what I'm thinking before I go to the appointment in the hopes that it will help me feel less flustered when we're there.  I don't like "first appointments" with doctors because I always get anxious and forget what I wanted to ask.  So, lists are a good thing.

What blood tests do you plan to do?
I was always frustrated that the only blood work my previous RE ever did was baseline FSH/LH but only after I forced her to do it about 5 months into treatment.  (She did TSH then, too, but there was little point in that since I'd had it done recently.)  When the FSH level came back "borderline high," she said she wanted to be more aggressive because of it but then never was and that was a year ago.  Also, if they are coded as "screening," blood work is more likely to be covered by my insurance.  So, whatever you can test that way, do it! 

Do you think my left tube has any issues?
My previous RE originally looked at my HSG results and said she thought there might be scar tissue at the end of my left tube.  After opening the right tube with a hysteroscopy (we know it's open, now, because I O'd from there when I got pregnant) she said all should be fine with the left and no lap was needed to check it out.  When we were starting to talk about IVF four months later, she said that she thought the left tube might be forming a hydrosalpinx and wouldn't do IVF without checking it out with a lap first (because hydrosalpinx can cause fluid to go back into the uterus).  Looking at the HSG images, it looks nothing like a typical "sausage tube" hydrosalpinx.  I'd really like a second opinion on this.  Hopefully it won't involve getting another HSG, though, because that was incredibly painful for me.  And expensive.

Do you think there is any pregnancy loss blood work we should do?
I know I've only had one m/c and most likely it was a genetic issue, but I really don't want to find out after another one that a simple change could have made a difference.  If it takes another year to get pregnant, I don't think I can handle it if I lose another pregnancy and then find out there was an obvious problem.

How many "tries" of each new treatment do you do before moving on?
My previous RE was of the opinion that you should try each new thing 6 times before moving on.  That'd be great if I were 25.  I'm not.  Time is a bit more of the essence here. 

What is your opinion on injectable IUIs vs. IVF?
At this point, I don't know where I stand with this.  The one thing I'm pretty sure of is that I don't want to go back to Femara-IUIs.  I just don't see the point.  But, I'm torn between the "eh" success rates plus higher risk of multiples but cheaper vs. better success rates but crazy expensive option of IUI vs. IVF.  This new RE has much higher success rates (and numbers of cycles) for IVF than my previous RE for my age group.  Before I got pregnant, I was heavily leaning toward IVF as opposed to inject IUI.  So was my DH.  Now I think I still am, but I'm pretty sure he isn't.

Do you have discounts for self-pay?
Probably should ask this one first.  My previous RE had great discounts for self-pay for ultrasounds and IUIs and, if this new office doesn't, I'm pretty sure we're going to have to crawl back to my old RE no matter what.
I can't think of much else.  I honestly just want to get on with it already and do something.  I just want to feel more secure with the new doctor than the old one.  I want to feel like he's paying attention and taking me seriously.  I want to feel like he at least bothers to look at my file before talking to me and is consistent when we speak.

Of course, I won't know most of that at the first meeting.  But, really, at this point if I just had a plan that seemed realistic, I'd be good.

Meanwhile, I just gave my first final of the week.  Time to focus on grading for a while to distract me.

  1. I think all these questions sound good! I feel so much more confident going into a meeting like that when I am prepared and have all my questions written down. Good for you for doing this! Good luck on your appointment - I'm excited to hear how it goes!

  2. I also have been paying for my own iui's. $2,000 New Zealand dollars a time. So have already racked up $6,000. Im only doing to 3, then moving onto IVF. Which I will also pay for myself. Very stressful as will have to get a bank loan. My opinion if you can would be to do IVF. and really at the end of the day who cares about money. you can make more money. A baby will be well worth it. good luck and hope to read some good news for you soon.

  3. I like your list. Perhaps with regards to testing, you want to have follow-up questions. If you know what you want the answer to be, then you can push for it.

  4. I think those are all great questions. I had borderline high FSH, too, and my RE suggested 3 Clomid IUIs then IVF because, like your former RE said, she wanted to be aggressive. The fourth IUI we ended up doing was at my own insistence. My RE was of the opinion that injectible IUIs were not for the likes of me, but I know that other REs feel differently. I'm interested to hear what your new RE has to say.

  5. That sounds like a good list. IMO IVF is probably the way I'd jump next. You've done 4 IUIs and from the data my RE showed me IUIs usually work in 3-4 cycles if they're going to. But with that said IVF is a big financial and emotional commitment and if you and the Mr aren't of the same mind set maybe it would be worth another IUI or two to get on the same page?   6. That's a good list. You should definitely ask for some basic antiphospholipid antibody screening tests or ask if you can be put on low-dose baby aspirin your next pregnancy. As you say, you've only had one miscarriage, but that's one too many. And the testing really isn't that invasive and it's a really common problem.
    Also ask about progesterone supplementation. It's another "easy fix" for a few miscarriage causes.
    Honestly I'd go with the IVF if you can afford it because if you have tube issues it skirts the issue for the most part.

  7. Great list! Our RE was of the 3x persuasion, as odds do not increase. What clinic will you be going to? And maybe IVF will be the way to go, potentially leaving you embryos for an FET. Just a thought. I am curious to hear what the outcome is!

  11. Your list looks great. I'm keeping my fingers crossed for a good appointment (and a good doctor who actually listens to you). I'm scared of IVF myself, so depending on the cost differential, I might try at least one cycle of IUI before moving to IVF. New doctor, maybe a new result with IUI. I'm shocked that your old RE did 6 cycles before moving on. That seems really high to me - mine only does 3 and then pushes people pretty hard to move on to the next thing. That approach makes a lot more sense if you aren't paying out of pocket, though. So depending on how you feel about IVF, there's nothing wrong with going straight for it if the clinic's numbers are good. Either way, we're here for you <3

  13. It's crazy how we all go around in circles with feeling like we want to take a break, move forward, stick to our RE, go somewhere else, etc. It's a tough battle we're fighting, and I am impressed that you're going in with a battle-plan. Good for you!

  15. your list is excellent -- very thorough and you should get the answers that you are looking for. i totally understand what you mean by not remembering and then smacking yourself for forgetting to ask or mention something.

    i have a good feeling about your new RE and what the new year will bring for you. and you're so right about the bloodwork for m/c loss -- i had to bascially demand my GP to do the b/w for the blood clotting problem (APS) and then i was like, "well it is well-known that if you have ONE auto-immune disease that you have a higher chance of having ANOTHER one" -- and she was like, "mmm, i guess you're right" -- just do the damn test! it was easy and it came back negative..but sheesh, i don't know why they don't do them as standard seeing as all it is is collecting a little blood.

    good luck tomorrow! i can't wait to hear how it all goes (and i want a description of the doctor and his/her room too please! :D )

  17. Great list of questions! Like you, I need to write things down or I get rushed and forget. I might ask about baby aspirin as well, my RE uses it for anyone with a history of miscarriage. I hope you get all the answers and attention you need from the new RE and everything is a huge success. I'm thinking of you!
